Invoicing and reporting
ICBC Fee Guide for Health Care Providers
Nurse practitioners who complete a report requested by ICBC must also include an invoice in order to receive payment. You may use the health service provider invoice (CL392) or your own invoice format.
All invoices must include:
ICBC claim number
Date of accident
Patient name
ICBC can not process payment if any of the above information is missing.
Please ensure reports are shared with ICBC only where patient consent has been granted or where requested under legislation. The CL489 series of reports may be requested by ICBC under Section 28/28.1 of the Insurance (Vehicle) Act.
